Include the complete information of the requested work-author, title, sections/pages to be copied or reprinted, and number of copies to be made-along with a brief description of your project. Please note that no more than 25% of text pages from available titles may be used for course reprints.

- Complete contact information of requester including name, address, state, zip, country, email, phone, and fax;
- USIP Press title information including name of book, author, ISBN, chapter title /author where applicable, and page numbers;
- and, information about the proposed publication and specific rights requested including title, author/editors, publisher, formats requested, relevant prices and ISBNs, page count, pub date, print run(s), distribution territory, and languages requested.
